During your visit, you will discuss your concerns with Dr. Mishra. If indicated, testing such as skin prick testing may be performed. This helps develop a personalized treatment plan tailored to your needs.

Avoid long-lasting antihistamines (e.g., Reactine, Aerius, Claritin, Blexten, Rupall, Benadryl, Allegra) for 7 days before your appointment. Also, stop nasal sprays containing antihistamines, such as Dymista and Ryaltris, for 7 days prior to your appointment. Other medications such as steroid asthma puffers, topical steroids, and prednisone may be continued.

All new patients require a referral from a healthcare practitioner in Ontario. Visits are by appointment only, and no walk-ins are accepted. Cancellation fees apply for missed appointments or those canceled with less than two business days’ notice: $100 for new consultations and $100 for follow-ups. Virtual appointments are available when appropriate.

A valid OHIP card is required at each appointment. Expired or invalid cards will result in direct charges for the appointment.

Uninsured services, such as missed appointment fees, certain forms, and letters, are not covered by OHIP. This includes insurance forms for biologic treatments. Fees for these services are based on the 2025 edition of the OMA Physician’s Guide to Uninsured Services. Your insurance company may request a ‘drug special authorization request form’ be completed. The recommended fee for this form is $122.

You must have a referral faxed to Dr. Mishra at 519-746-1982 by a healthcare practitioner in Ontario.

You must follow-up with Dr. Mishra twice yearly in order to continue receiving prescriptions for injections, including at minimum one visit in-person. Conversations with patient support programs do not constitute a visit with my office.
